Full time: 8 A. M. – 5 P. M.

Position: 2820

Salary: $14.07-$17.31per hour; DOEEQ

Essential Duties and Responsibilities:

Duties may vary based on specific program and position, and may include but are not limited to the following:

· Completes intakes for services timely and makes referrals within the organization or to other agencies that can meet the individual’s needs.

· Coordinates diagnostic testing and the determination of eligibility of IDD and related conditions for individuals requesting services.

· Maintains a centralized comprehensive interest list for IDD services in accordance with Texas Health & Human Services (HHSC) and LIDDA standards and procedures for the Home and Community Based Services (HCS) Medicaid Waiver, Texas Home Living (TXHML) Medicaid Waiver, and General Revenue (GR) programs. Communication and interaction with consumers, staff and community individuals.

· Conducts biennial contact with individuals or their primary correspondent who are listed on the interest list depending on program type. Maintains 100% on interest list biennial contact and date entry.

· Assist individuals seeking ICF placement.

· Assist with waiver enrollments as needed.

Competencies

The competencies listed below are representative of the knowledge, skills, and abilities required to successfully perform the essential functions of this position:

· Working knowledge of English language (spelling, grammar, punctuation) required.

· Skilled use of Windows and Office products (Microsoft Word and Excel essential) Internet, and other related office equipment is also required

· Must possess quality, positive people skills.

· Must possess the ability to multitask, manage time, and independently organize the multiple demands to meet deadlines.

· Ability to communicate effectively both orally and in written form.

Education and Experience:

· High school diploma or GED required.

· Prefer a bachelor’s degree in a human service field.

· Experience in field of intellectual disabilities desired


Certifications, Licenses, and Registrations:

· Valid Driver’s License required
