Position: 2820

Salary: $14.07-$17.31per hour; DOEEQ

Essential Duties and Responsibilities:

Duties may vary based on specific program and position, and may include but are not limited to the following:

· Under the moderate supervision of the Chief of IDD and with limited latitude for initiative and independent judgment, the IDD Intake Specialist serves as the identified point of contact for individuals, families, professionals, and agencies seeking information regarding Intellectual Development Disability (IDD) services in the 7 county local service area of Center for Life Resources.

· Completes intakes for services timely and makes referrals within the organization or to other agencies that can meet the individual’s needs.

· Coordinates diagnostic testing and the determination of eligibility of IDD and related conditions for individuals requesting services.

· Maintains a centralized comprehensive interest list for IDD services in accordance with Texas Health & Human Services (HHSC) and LIDDA standards and procedures for the Home and Community Based Services (HCS) Medicaid Waiver, Texas Home Living (TXHML) Medicaid Waiver, and General Revenue (GR) programs. Communication and interaction with consumers, staff and community individuals.

· Conducts biennial contact with individuals or their primary correspondent who are listed on the interest list depending on program type and complete data entry.

· Assist with individuals seeking ICF placements.

· Assist the Enrollment Specialist with Waiver enrollments, transfer enrollments, and crisis diversion slots.

· Attend resource fairs to help promote IDD services within the community and local ISD’s.

· Skilled use of Windows and Office products (Microsoft Word and Excel essential) Internet, and other related office equipment is also required

· Must possess the ability to multitask, manage time, and independently organize the multiple demands to meet deadlines. Strong organizational and time management skills.

· Ability to communicate effectively both orally and in written form.

· Ability to collaborate effectively with diverse individuals, interdisciplinary teams, and systems.

· Proficient in accurately and objectively recording data and documentation.

Education and Experience:

· Prefer a bachelor’s degree in a human service field.

· For non-degree prefer at least two years of experience in field of intellectual disabilities along with High school diploma or GED.


Certifications, Licenses, and Registrations:

· Valid Driver’s License required
