MI PASRR Specialist (QMHP-CS)
Overview:

As a MI PASRR Specialist, you’ll help individuals entering nursing facilities connect to the specialized services they need—while obtaining or building on real‑world skills in assessment, recovery planning, interprofessional collaboration, and documentation across multiple platforms.

Essential Duties and Responsibilities:
Duties may vary based on specific program and position, and may include but are not limited to the following:
· Complete timely PASRR assessments for individuals seeking admission to nursing facilities and determine eligibility for PASRR specialized services.
· Enter and maintain accurate records in the Electronic Health Record (EHR) and within required state systems (e.g., TMHP Long‑Term Care portal), meeting all HHSC/CFLR timelines.
· Manage a person‑centered caseload; complete strengths‑based assessments (e.g., ANSA/CANS) and keep Recovery Plans current.
· Provide skills training, linkage, and ongoing monitoring to help individuals achieve recovery goals and remain engaged in care.
· Document all services accurately and on time in the Electronic Health Record (EHR) and per HHSC/Medicaid/CCBHC requirements.
· Collaborate closely with clinics, hospitals, primary care, nursing facilities, and community partners to reduce barriers to care.
· Maintain compliance with HIPAA, TAC Title 26, and internal policies; contribute to quality improvement initiatives.
· Uphold CFLR standards for professionalism, teamwork, initiative, and dependable follow‑through.

· Bachelor’s or Master’s degree in a Human Services-related field such as:
o Psychology
o Sociology
o Social Work
o Rehabilitation
o Counseling
o Human Growth and Development
o Gerontology
o Special Education
o Educational Psychology
o Early Childhood Education
