Individuals have the right to file a grievance about a staff member, services, violation of individual rights, and\/or other concerns regarding Center for Life Resources or its Community Partners. Center for Life <\/span>Resources considers all complaints in a serious manner, making every attempt to resolve the issue <\/span>thoughtfully, objectively and without bias.<\/span><\/p>\n

\u2022 If an individual and the immediate staff are unable to resolve a complaint, the staff will refer the individual to the appropriate Program Supervisor. If the Program Supervisor is unable to resolve <\/span>the issue then the Client Rights Protection Officer will step in to assist. The Client Rights <\/span>Protection Officer is the client advocate and liaison to Center for Life Resources.<\/span><\/p>\n

\u2022 The Program Supervisor, Client Rights Protection Officer, and\/or the complainant may call a meeting of all concerned parties to resolve the problem. If so, the Program Supervisor will <\/span>arrange such a meeting, document the results, and have all acknowledge participation.<\/span><\/p>\n

Cost of Services<\/strong><\/span><\/p>\n

Center for Life Resources charges reasonable fees for services and will not deny services to a person because of an inability to pay. Discounts for essential services are offered based on family size and income. Center for Life Resources uses the Health and Human Services Monthly Ability-to-Pay schedule to determine an individual\u2019s ability to pay<\/p>\n

The Sliding Scale charts below determine your maximum monthly fee based on several factors, including your household size, your annual or monthly gross income, and any extraordinary expenses (such as medical expenses or property loss). It is best to utilize this chart as a tool, as clinic staff will discuss this with you at the time of service enrollment.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Good Faith Estimate<\/strong><\/span><\/p>\n

If you are uninsured and do not qualify for a reduced fee through the monthly ability to pay, you will receive a good faith estimate of what could be the cost for your mental health services for 12 months. This does not include the cost for unscheduled or crisis services.<\/p>\n

If you have insurance and choose to not use your insurance benefits to pay for services, you will receive a good faith estimate.<\/p>\n
